ACLS for Paramedics
Paramedics manage cardiac arrests with fewer hands, less space and more distractions than a hospital team. An ACLS course gives them the same algorithms, applied in the conditions of the street and the ambulance.
In short
Paramedics who carry a manual defibrillator and resuscitation drugs need ACLS, usually with PALS. Field priorities: high-quality CPR with minimal interruption, early defibrillation, capnography, and early recognition of STEMI and stroke for the right hospital.
The algorithms are the same; the constraints are different. Two or three clinicians must cover compressions, airway, defibrillation, drugs and communication. Mechanical aids, clear roles and a practised sequence make the difference — see the Megacode guide for how team leadership is taught.
ACLS needs current BLS. Because the next call may be a child, most ambulance services also require PALS.

Moving a patient in arrest reduces CPR quality. The course discusses when to stay and work on scene and how to plan extrication so that pauses are short. Mechanical CPR devices, where available, help during transport.
A structured handover at hospital — rhythm history, shocks, drugs, times and suspected cause — lets the receiving team continue without repeating work.
