ACLS Megacode: How It Works and How to Pass as Team Leader

The Megacode is the practical heart of the ACLS course: a simulated emergency in which you lead the team through the algorithms. It is where knowledge turns into leadership.

What happens

A scenario begins, often with a patient who is still conscious, and evolves: a bradycardia or tachycardia, then arrest, rhythm changes, and return of circulation. You assign roles, make decisions and direct the team. See the pretest and exam for how it fits with the other tests.

What instructors look for

• The systematic approach, said aloud — systematic approach.
• Correct algorithm decisions in order — ACLS algorithms.
• High-quality CPR with minimal interruptions.
• Correct drugs and electrical therapy at the right time — drugs, electrical therapy.
• Clear roles, closed-loop communication and considering reversible causes.

How to lead

1. Assign roles at the start by name.
2. Say what you see: “This is VF.”
3. Give one clear instruction at a time to a named person, and wait for confirmation.
4. Watch the clock: rhythm checks and adrenaline timing.
5. Summarise every few minutes and ask the team for ideas.

Common mistakes

• Doing tasks yourself instead of leading.
• Forgetting to resume compressions after a shock.
• Giving a drug for the wrong pathway.
• Not recognising return of circulation.

Rehearse with practice questions and team dynamics. If you do not pass first time, remediation and a retest are part of the course.
